Application Form

Our mission is to provide interest-free loans up to $5,000 to members of the Jewish communities of Cincinnati and Dayton.

To be considered for a loan, the first step is to submit an application and a supporting application from your loan co-signer. (See links below.)

Once we receive both the borrower and co-signer application forms, we will do everything we can to streamline the review process.

We will contact you to set up a meeting. Currently, meetings are conducted via Zoom.

After your loan is approved, we will work with you to develop a customized payment plan.

Before You Start

Recommended browsers: We recommend using Google Chrome or Firefox browsers to open and complete the application. The forms are also user-friendly for mobile devices.

Gather supporting documents: As part of your application, you’ll be asked to scan and upload these documents:

  • Copy of most recent pay stub or other proof of income
  • Documentation of current year-to-date income, if possible

Saving the form before submission: Click on the link below for instructions on how to save the form while you are in the process of filling it out.

Submitting the form: To submit the form, you will need to complete all required fields and sign (electronic signature) the form. After you submit it, you’ll receive a confirmation email summarizing the information you’ve provided.

1. Save and Continue Later

Scroll down to the bottom of the page you are completing. Click Save and Continue Later. 

2. Saved Submission

After you click Save and Continue Later this window will pop up. The email that you provided at the top of the application will automatically pop-up. Click on Get Shortcut Link. (You may get a message that asks to copy the link.) Copy the link and save it. 

3. Discard or Continue Application

When you return to complete or add to the form, a window will pop-up. The message will prompt you to discard the information that you have entered on the application or continue where you left off. 

Borrower's Loan Application

Borrower:
Click on the button below. A window will pop up on your device. Complete the application. All parties must sign the application electronically. Gather the documents from the list and upload them to the application. Contact your co-signer(s) to complete his/her portion of the application.

Click on the button below. A window will pop up on your device. Complete the application. Sign the application electronically. Gather the documents from the list and upload them to the application. Submit the completed application and uploaded documents.
